Before getting a lawyer, just start getting names. The next time you get Helpful Person on the line, ask her name and write it down. Also note the time of the phone call. You often get better results by saying "Jennifer told me on May 14 at 10:32am that this issue had been resolved." Also, if you DO wind up having to get a lawyer, a written log of the drama helps immensely.

Actually, especially given the effect making such phone calls has on my blood pressure, I should probably send a carefully-crafted angry letter, by certified mail with return receipt. That's usually good for getting someone's attention.
It's a good thing I didn't manage to get away for a vacation this month, or the unexpected bill would have waited and waited and....
